29 CFR 1910.147(a)(2)(ii)(B): An employee was required to place a part of his or her body into an area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle. In production F4, the employer did not ensure the batter mixer 4 machine was de-energized prior to maintenance being performed exposing the employee to amputation hazards.
