29 CFR 1926.552(c)(17)(iv)(a): Wire rope shall be taken out of service when any of the following conditions exist: In running ropes, six randomly distributed broken wires in one lay or three broken wires in one strand in one lay: On or about October 20, 2017, during the construction of an underground sewer relief tunnel the employer did not remove from service a wire running rope missing an entire rope strand. The wire rope was wound on the drum of a hydraulic powered pull master winch and used to hoist material.
