29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation: The employer is failing ensure that employees are protected from amputation hazards. This most recently occurred at 6316 John J. Pershing Drive, Omaha, Nebraska 68110 on October 17, 2017. The employer failed to ensure that adequate machine guarding was provided on the A 40 bracket welder. An employer reached into machine to clean an index sensor. The machine actuated, causing an amputation of the employees finger.
