29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. An examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified. Specifically, reviews were not conducted to determine if listed steps were still adequate.
