29 CFR 1928.57(c)(4)(i): Guards, shields, and access doors shall be in place when the equipment is in operation. The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and amputation hazards created by rotating parts, flying chips and sparks: The employer is failing to protect employees from struck-by and amputation hazards while engaged in machining and maintenance activities in the shop area. This was most recently documented on or about October 24, 2017, at the jobsite located at 204 SE 70th Avenue Stafford, KS 67578. The employer did not ensure that machine guarding was provided and utilized. Employees were not provided protection from flying chips and debris in that the rotating chuck was not guarded for the Grizzly Mini Milling machine (Lathe). Among other methods, a feasible and acceptable method of abatement includes the installation of machine guarding as required in ANSI B11.8-2001 (R2012) - Safety Requirements for Manual Milling, Drilling & Boring Machines with or without Automatic Control.
