29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On 4 October 2017 at approximately 11:30 AM, an employee with the company struck his head against a Hitachi Compact Excavator 35U bucket while wearing head protection, suffering an injury to the back right side of his head. The employee complained later in the day of not feeling well with nausea, vomiting and headaches.
