29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. On or about October 17,2017, employees were exposed to being struck by and trench cave in hazards. Employees were involved in gas line repair at 3913 Darrow Road, Stow, Ohio 44224. An employee was working in a trench that was Approximately 5 feet 5 inches deep; the excavated materials were not at least 2 feet back from the edge of the trench.
