29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Fabricating Department - on or about October 5, 2017, the employer did not ensure shutdown of the Automatan Label Laminator, Machine number 501 was conducted in accordance with appropriate documented company procedure when employees un-jammed labels, resulting in an amputation.
