29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about September 26, 2017, during the installation of a light fixture at a job site located at 400 Nave Rd SE, Massillon, Ohio 44646; the employer did not ensure employees working on the roof of a new construction work bay "tunnel" were protected from a fall hazard of approximately 10.6 feet through a roof opening approximately 2 foot by 4 foot by utilizing a fall protection system
