29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. On or about 27 September 2017, the following necessary steps to protect the employees from unexpected energizing, startup or release of stored energy were not accomplished: a. The employer did not establish an energy control (lockout/tagout) program consisting of energy control procedures to ensure that before any employee performed any servicing or maintenance, the machine would be isolated and rendered inoperable. Employees exposed to caught-in hazards likely to result in crushing injuries and/or amputation from unexpected energizing, start up or release of stored energy from the bowling pin machines. b. The employer failed to effectively train employees to identify each potentially hazardous energy source and isolate the source(s) during servicing and maintenance operations on all company equipment, including the Flood Coater. This exposed employee(s) to amputation and/or crushing injuries from unexpected start-up. c. The employer did not conduct a periodic inspection of the energy control procedures at least annually to ensure that the procedures and the requirement of this standard were being followed. Employees utilizing undocumented procedures were exposed to caught-in hazards and amputation hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
