29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: A) Uni-Grip, Inc., located at 9823 State Highway 53 North, Upper Sandusky, OH 43351: On or about September 21, 2017, the employer did not establish a lockout/tagout program for the control of the hazardous energy, which includes procedures, training, and periodic inspections, while performing activities such as, but not limited to, conducting set-up of the equipment such as, but not limited to, the hole puncher machine (built in-house). Employees were exposed to amputation hazards.
