29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of struck by hazards related to positioning piping within the confines od a trench: (a) Job Site at 110 Pitney Road, Lancaster, Pennsylvania - The employer did not instruct employees in the recognition and avoidance of caught-in-between and lateral struck-by hazards that exist when employees work in a trench where materials and equipment get moved laterally, thereby exposing employees to these hazards. On or about September 25, 2017, employees worked centering a newly laid pipe in a trench in an area where existing piping crossed the trench perpendicularly. When the backhoe operator applied lifting pressure to the lifting sling attached to the newly laid pipe to assist the employees in centering this pipe, the pipe came free from the joint, swung laterally, striking an employee's lower leg and pinching it against the existing pipe that ran perpendicularly across the trench. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
