29 CFR 1910.212(a)(3)(iii): The employer did not ensure that special hand tools for placing and removing material(s) was provided to permit easy handling of material without the operator placing a hand in the danger zone: The employer is failing to protect employees from amputation hazards associated with working near hazardous points of operation on meat packing equipment. The most recent occurrence of this was evident on September 15, 2017 during normal operations at the facility located at 13075 Renfro Circle, Omaha, NE; where the employer has an employee reaching to pull slabs of beef off the skinner machine without using a tool, resulting in the employee having his finger amputated by the rotating skinner blades.
