29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Colorado Petroleum Products Company at 5590 High Street, Denver, CO, 80216: On or before September 20, 2017 the employer did not ensure the point of operation of the Pasco Palletizer 6650 was properly guarded to prevent employees from having any part of the their body in the danger zone during operation. On September 18, 2017 an employee gathering boxes on the line sustained serious injuries when he was struck by the wiper arm after it activated. The employee accessed an unguarded section of the machine which was opposite the control panel. This condition exposed employees to caught-in-between, struck-by, and crushing hazards.
