Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from the hazards such as those created by point of operation, in-running nip points, rotating parts, flying chips and sparks: The employer is failing to protect employees from amputation hazards associated with packaging machinery in the facility. Instances in which the machines were in violation of this standard are as follows: a) An employee suffered a partial amputation of their finger on the TEXWRAP #2 machine on September 1st, 2017. b) An employee suffered a partial amputation of their finger trying to unjam the packaging machine on the Red Line on September 23rd, 2017. Employees are required to operate these machines with either no guard covering the point of operation or a portion of the point of operation not fully guarded allowing employees to contact the rotating parts creating a laceration/amputation hazard. This was most recently documented at 4651 F St., Omaha, Nebraska on or about September 12th, 2017 when the employer had employees working on the above listed machines without proper machine guards.
