29 CFR 1910.28(b)(3)(i)(A): The employer did not ensure that each employee on a walking-working surface was protected from falling into any hole or pit that was 4 feet (1.2 m) or more above a lower level by a cover or covers: The employer is failing to protect employees from tripping and fall hazards associated with grain elevator housekeeping activities. This was most recently documented, on 09/19/17, at the establishment located at 1892 US Hwy 83, Sublette, Kansas. The employer did not assure covers were readily available for all floor holes on the distribution level of the grain elevator. Employees engaged in housekeeping activities were exposed to trip and fall hazards into open distribution level floor holes.
