29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question: a. On or about September 15, 2017 at RCM Interiors, LLC while dismantling a tube and coupler scaffold on the east side of a building renovation, the employer did not ensure that employees successfully completed training to recognize hazards associated with dismantling the scaffold, exposing employees to struck-by, crushing, and a 7 feet 8 inch fall hazard.
