29 CFR 1910.269(d)(6)(ii): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown was not used to avoid any additional or increased hazards to employees as a result of the equipment stoppage. At a substation in Hamilton, Texas, employees worked on an energized re-closer rated at 7,620 volts that was not shutdown or de-energized as required by Brazos Electric Power Cooperatives procedure prior to performing maintenance exposing the employees to electrocution hazards.
