Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from struck by hazards: (a) Facility, 595 Plant Road, New Florence, PA: On or about August 31, 2017 the JCB 4CX15-T backhoe was being used with an attachment installed that had not been approved for the specific machine by the manufacturer. The attachment is approximately 17'5" in length and was fabricated at a local welding shop. An employee was injured when the attachment struck the operator's cab, broke the window and hit the employee on the right side of their face and head. The employee sustained head trauma and a broken right orbital bone.
