Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. a. On or about August 25, 2017, a Pan-O-Mat machine operator suffered the amputation of the tip of their right middle finger while scraping dough from the inside of the machine's hopper prior to a scheduled break. Oil from the dough had coated the scraper handle causing the scraper to slip from the employee's hand. The amputation occurred as the employee reached into the energized machine's hopper to retrieve the scraper. The employer failed to implement energy control application steps in order to perform the cleaning work per the requirements. As a result, the remaining applicable energy control elements required by 1910.147(d), involving machine isolation [(d)(3)], LO/TO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. Alfred Nickles Bakery, Incorporated was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(d)(2), which was contained in OSHA inspection number 943006, citation number 1, item number 1b and was affirmed as a final order on March 27, 2014, with respect to a workplace located at 1950 N. Sugar Street, Lima, Ohio 45801.
