OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a caught between and/or amputation hazards while operating a press brake with a defective/damaged foot control/pedal: On or about August 09, 2017, the foot pedal used to actuate the company's Cincinnati: 90 ton press brake was defective in that the screws used to secure the protective cover to the foot control were missing.
