29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): (a) Complete Energy Services, Inc. DBA A & W Water Service Inc. Located at CR 64 and 61.5 Gill, CO 80624: On and preceding August 24, 2017, the employer did not certify that operators of rough terrain forklifts had been trained and evaluated in accordance with the OSHA standard. The company used rough terrain forklifts, such as but not limited to, a JLG 12-55, to transport hose spools, and other materials and equipment on job sites and through open fields. Written certification was not completed for this employee training on the rough terrain forklifts.
