Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Employees who perform servicing and maintenance on Centrifuge Dryers were not required to; 1. Notify affected employees of the servicing and maintenance on the dryer. 2. Turn off and shut down the machine. 3. Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source. 4. Affix lockout devices to electrical and pneumatic energy sources. 5. Prior to starting work on machines that have been locked out, verify that isolation and deenergization of the machine have been accomplished. On August 18, 2017 in the Dryer Room Area, Deck 5, Dryer #8, a maintenance employee was exposed to an amputation hazard while inspecting a pulley and belt on a Heinzen Centrifuge Dryer when he moved his hand up underneath where a pulley and belt were located and about this time the dryer machine started operating.
