On or about August 18, 2017 at 136-40 Springfield Boulevard, Springfield Gardens, NY 11413 (Local Sort Department) The employer failed to provide affected (i.e. singulator belt tender) employees effective hazardous energy recognition and machine operation training which resulted in the amputation of an employee's finger tip.
