OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to a struck by hazard from towing devices and equipment used in pulling heavy trucks and equipment. (a) Plymouth Sorrento Road, Apopka, FL - on or about July 18, 2017, only one of the two tow hooks at the rear of the Vac-Con vacuum truck was used to attach a 20 foot long by 4 inch wide nylon sling that was wrapped around the front rail of the Caterpillar 938H Series loader in order to pull the vacuum truck from a ditch where the right front tire had gotten stuck.
