29 CFR 1910.178(m)(10): Load backrest extension(s) were not used on industrial trucks to minimize the hazard of rearward falling materials: a) Warehouse, Shipping and Receiving; On or about August 21, 2017 and at times prior, the employer exposed employees to struck-by and crushed-by hazards in that employees were allowed to use step-on style "Walkie Rider" pallet jacks without a load backrest.
