29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: On or about 08/08/2017, a worker was pinned under fertilizer hopper that was being lifted by a Doosan powered industrial truck Model No. DL 250-3 with a bucket loader attachment The worker sustained crushed pelvis bones and bruised ribs. The load was not stable.
