Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to hazards associated with not securing propeller fins during carbon arc cutting: a) Saint Julien's Creek: On or about July 12, 2017, an employee was carbon arc cutting a propeller fin when the fin fell striking and crushing the employee causing multiple injuries. Employees worked with and around unsecured propeller fins during removal in the following manner: 1) Employer did not develop and implement work rules, job hazard analysis, or conduct a hazard assessment for PPE and effectively train employees in safe procedures for removing the fins. 2) Employees worked with fins that were not secured with supports or some other methods during cutting. Employees worked within the fall shadow of the fins.
