29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service. (a) BB&T Bank: On or about August 9, 2017 and times prior, the Fall Safe fall arrest harness that was provided by the company was visually inspected but not removed from service due to visible deterioration, such as rusted metal buckles and missing labels.
