29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) C2 Imaging, LLC: On or about August 9, 2017, and at times prior, employees were exposed to amputation or laceration hazards due to lack of adequate guarding on the point of operation of the Rosenthal Sheeter Model material cutter. (b) C2 Imaging, LLC: On or about August 9, 2017, and at times prior, employees were exposed to crushing hazards due to lack of adequate guarding on moving parts (such as the cam shaft) of the Stimpson 83GW Grommetter. (c) C2 Imaging, LLC: On or about August 9, 2017, and at times prior, employees were exposed to in-running nip points due to bypassing a light curtain sensor on the Orca Pro-Tech Laminator.
