29 CFR 1910.119(e)(3): The process hazard analysis did not address the elements of (i) through (vii) of this paragraph: The employer is failing to protect employees from explosion and fire hazards by failing to evaluate the hazards of the lead styphnate manufacturing process in the Process Hazard Analysis. This was most recently documented at 25201 E. 78 Hwy, Independence, Missouri. The employer's process hazard analysis did not address the effect of loss of video surveillance on the lead styphnate manufacturing process.
