OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and struck-by hazards from wood frame structural collapse: a) On or about August 2, 2017, and at times prior thereto: Employees were constructing a wood frame building with an eave height of 22 feet and 82 feet long trusses. The roof was overloaded with 5/8 inch oriented strand board (OSB) sheathing in two (2) separate stacks 32 and 40 inches. b) On or about August 2, 2017, and at times prior thereto: Employees were constructing a wood frame building with an eave height of 22 feet and 82 feet long trusses. Wall bracing was insufficient consisting only of exterior ground diagonals on the northwest, northeast, and southeast exterior walls. The southwest side of the structure had only interior ground brace diagonals anchored to the concrete floor of the structure. c) On or about August 2, 2017 or prior thereto: Employees were constructing a wood frame building with an eave height of 22 feet and 82 feet long trusses. The wood trusses were 2:12 slope. Only six (6) out of forty seven installed trusses had temporary diagonal bracing located near the southwest side of the structure. d) On or about August 2, 2017 or prior thereto: Employees were constructing a wood frame building with an eave height of 22 feet. The 82 feet long trusses had been lifted into place using a spreader bar only 42 feet long. NOTE: Abatement certification AND supporting documentation are required for this item.
