29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity: a). On or about July 29, 2017 grounded conductors of a residential 120 volt circuit and a dishwasher power supply circuit were attached in a manner that reversed the designated polarity of the circuits exposing employees to electrocution hazards.
