29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) On or about July 20, 2017, the employer failed to record an illness on the OSHA 300 Log and failed to fill out a 301 Incident Report when an employee, working in extreme heat conditions, suffered from and received treatment for symptoms of heat related illness experienced first on or about June 19, 2017, and again on or about July 7, 2017. In each incident the illness resulted in days away from work. b) On or about July 27, 2017, the employer failed to record an illness on the OSHA 300 Log when an employee, working in extreme heat conditions, suffered from and received treatment for symptoms of heat related illness experienced on or about July 18, 2017. The illness resulted in days away from work. c) On or about November 2, 2017, the employer failed to record an illness on the OSHA 300 Log when an employee, working in extreme heat conditions, suffered from and received treatment for symptoms of heat related illness experienced on or about June 24, 2017. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.4(a), which was contained in OSHA inspection number 950338, citation number 2, item number 1 and was affirmed as a final order on February 11, 2014, with respect to a workplace located at 401 W. Lexington Ave. El Cajon, CA 92020. Or, "IN THE ALTERNATIVE" 29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: a) On or about July 20, 2017, the employer failed to record an illness on the OSHA 300 Log and failed to fill out a 301 Incident Report when an employee, working in extreme heat conditions, suffered from and received treatment for symptoms of heat related illness experienced first on or about June 19, 2017, and again on or about July 7, 2017. In each incident the illness resulted in days away from work. b) On or about July 27, 2017, the employer failed to record an illness on the OSHA 300 Log when an employee, working in extreme heat conditions, suffered from and received treatment for symptoms of heat related illness experienced on or about July 18, 2017. The illness resulted in days away from work. c) On or about November 2, 2017, the employer failed to record an illness on the OSHA 300 Log when an employee, working in extreme heat conditions, suffered from and received treatment for symptoms of heat related illness experienced on or about June 24, 2017. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.29(b)(3), which was contained in OSHA inspection number 1075844, citation number 3, item number 1 and was affirmed as a final order on September 8, 2016, with respect to a workplace located at 616 W. 2nd St. Ottumwa, IA 52501.
