29 CFR 1910.30(a)(3)(ii): The employer did not train each employee on the procedures to be followed to minimize fall hazards: a) Twin View Farms, Inc. at 17868 County Road 15, Platteville, CO 80651: On and before July 14, 2017, the employer did not train each employee on the procedures to be followed to minimize fall hazards when working on top of a horizontal silage pile. Employees working on top of the pile used a hand knife to cut the plastic tarp away from the vertical face of the pile to allow a loader to remove the silage for feeding. Employees typically exposed about 2 to 3 days' worth of feeding at a time, or up to about 10 feet of tarp. Employee also cut the twine used to hold the tires on the tarp, and then threw the tires either back onto the pile, or over the front edge. Employees were allowed to approach within 3 feet of the front, top edge of the silage pile. This condition exposed employees to fall hazards of approximately 20 feet.
