29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: a) Facility: The employer failed to report the inpatient hospitalization of an employee to OSHA within 24 hours. The employee suffered a concussion from falling from a 6ft ladder and was hospitalized on 7/6/2017 and it was not reported to OSHA until 7/19/2017.
