29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) On or about 06/30/2017 at 1410 Pinehurst Drive in Dunedin, Florida, an employee fell from a roof causing trauma to the head and torso and was hospitalized. The employer was aware of the in-patient hospitalization on Friday, 6/30/17. The employer notified OSHA on 7/5/2017 at 5:02 pm of the in-patient hospitalization.
