OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by falling containers (dumpsters. (a) Employees were emptying the contents of rear loading containers (dumpsters) into the hopper of a 1995 White GM Rear-loaded Garbage Truck (VIN: 4V2JCBBE3SR837028) where trunnion bar latches were not serviceable or available for use, exposing employees to the hazard of being struck by a falling container. (b) Employees were emptying the contents of rear loading containers (dumpsters) into the hopper of a 1995 White GM Rear-loaded Garbage Truck (VIN: 4V2JCBBE3SR837028) did not lower the containers to the ground prior to engaging the hopper blade, exposing employees to the hazard of being struck by a falling container. Violation occurred on or about 29 June 2017.
