29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about 9:30 am on 06/17/2017 at 3226 Nautical South, Saint Petersburg Florida, an employee suffered a left thumb amputation as a result of a work related injury. The employer was aware of the injury in the morning of 06/18/2017. The employer notified OSHA on 07/06/2017 at 2:03 pm of the amputation incident.
