29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: (a) On or about June 17, 2017, at a jobsite located at 7950 Ohio Drive, Plano, TX, employees using a 2-inch hose to pump concrete into the holes of a concrete wall while working from a pump jack scaffold working platform that was not fully planked were exposed to a fall hazard.
