29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator had been involved in an accident or near-miss incident (a) On or about May 17,2017, the employer did not remove a forklift operator from performing forklift operation after being involved in an accident resulting in injury. Retraining of the operator did not occur to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely.
