29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question: a. On or about June 30, 2017 at Williams Industrial & Marine, Inc. while dismantling a tube and coupler scaffold on the south side of a new constructed cinderblock wall, the employer did not ensure that employees successfully completed training to recognize hazards associated with the dismantling a scaffold, exposing employees to struck-by, crushing, and a 13 feet 5 inch fall hazard.
