29 CFR 1926.950(d)(5): Existing characteristics and conditions of electric lines and equipment that are related to the safety of the work to be performed were not determined before work on or near the lines or equipment is started. Such characteristics and conditions include, but are not limited to the locations of circuits and equipment, including electric supply lines, communication lines, and fire protective signaling circuits: On or about June 26, 2017, and in times prior thereto, all phases feeding a two-way feed transformer were not verified as to their source and grounded before making physical contact with conductors exposing electricians to an electric shock and burns hazard. This condition existed at pad mounted transformer, located near 720 Mariner, Lakeway, Texas.
