29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) Jobsite, Southern Beltway, Section 55B, McDonald, PA: On or about June 21, 2017 cave-in protection was not provided for employees working in an excavation approximately 12 feet deep. A section of the upper west wall broke off and struck the employee, pinning him against a manhole. The employee sustained broken bones to the right metatarsal, left elbow, T12, L1 and L4 vertebrae.
