29 CFR 1910.138(b): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Cold Mill- 2626 Ligonier Street, Latrobe, PA: On or about June 26, 2017, employees were exposed to the rotating alloy stainless steel bar while being fed into the Medart Continuous Automatic Two-Roll Rotary Straightening, Sizing and Polishing Machine; causing two finger amputations.
