29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved: On June 19, 2017, the employer did not adequately instruct at least annually, every employee in the safe operation and servicing of machinery such as, but not limited to, the John Deere tractor and the Agro Baler, when used for baling hay on the hay farm located approximately 30 miles southeast of Del City, TX. Employees were exposed to struck-by hazards during the time the equipment was in operation.
