29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions: a) One Avenue L, Newark, NJ 07105 - MCC 35N Employees exposed to arc flash and shock hazards were not provided with Hazard Risk category (HRC) II arc rated shirt and pants or coveralls while working on 480 volt energized live parts. On 6/3/2017, an employee received second degree burns when an arc flash occurred. Violation occurred on or about 6/3/2017
