On or about June 19, 2017, at a jobsite located at 9000 S. Thomas Rd. Panama City Beach, FL. the employer did not ensure live circuits to which the employees was exposed to were de-energized while replacing a fountain lighting system, exposing employees to the hazards of electrocution.
