29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: A. On or about June 9, 2017 at 7:45pm, a warehouse associate operating a Clark high reach truck at a host site, sustained an injury requiring hospitalization. The injury was reported to OSHA on June 14, 2017, more than 24 hours after the incident.
