29 CFR 1910.119(e)(1): The process hazard analysis did not identify, evaluate, and address the control of the hazards involved in the process: (a) In the Facility, B-14 and B-10- On or about June 5, 2017, the employer did not evaluate the Rosedale filter system in the process hazard analysis (PHA) to identify the hazards related to the filter. The filter failed during the operation of Titan Step 3, allowing DCDMH to bypass the filter and enter the vacuum system and pump which caused the vacuum pump housing to explode, injuring three workers.
